What is the rotator cuff?
The rotator cuff is a group of four muscles and their tendons — supraspinatus, infraspinatus, teres minor, and subscapularis — that wrap around the head of the humerus (upper arm bone) and hold it firmly in the shoulder socket. They work together to rotate and lift the arm, and to stabilise the joint during virtually every upper-body movement.
Common rotator cuff problems
Rotator cuff tendinopathy
Tendinopathy is a degenerative change in the tendon tissue, typically caused by overload — either a sudden increase in activity or sustained repetitive overhead movements over time. It presents as a deep, aching pain in the shoulder, often worse with lifting the arm to the side or overhead.
Rotator cuff tears
Tears can be partial or full thickness. They can result from an acute injury (such as a fall on an outstretched hand) or gradually develop from long-standing tendinopathy. Interestingly, many partial tears are asymptomatic — imaging findings don't always correspond to pain levels, which is why a thorough clinical assessment is so important.
Shoulder impingement
Impingement describes the painful pinching of rotator cuff tendons under the bony arch at the top of the shoulder (the acromion). It's often associated with poor scapular control, muscle imbalances, or postural habits that reduce the space available for the tendons to move.
